First Name can be no longer than 50 characters and can include only numbers, letters, and common punctuation symbols. The entry for First Name cannot contain reserved characters such as $ or }.

Last Name can be no longer than 50 characters and can include only numbers, letters, and common punctuation symbols. The entry for Last Name cannot contain reserved characters such as $ or }.

E-mail must be in a valid format that includes a username, followed by the @ symbol, followed by a domain name. There is no test of whether the email address exists or works, so check to make sure your entry is correct if you wish to receive a response.

Phone can be left blank if you do not wish to be contacted by phone. If there is an entry for Phone, it must be a number for the USA or Canada that includes an area code and a seven digit number (e.g. 555-123-4567). This can be followed, optionally, by an extension or label of up to 10 numbers and letters. (Note: If you are outside the USA or Canada, we will be glad to contact you by email.)

Please contact me allows you to use check boxes to indicate whether you wish to be contacted by E-mail or by Phone. The box for contact by E-mail is checked automatically. If you wish, you can uncheck this and check the box for contact by Phone, or you can check both boxes to request contact by either method.
